How would an exception be recorded and escalated?

Create a small working record with inputs, reasoning, evidence and checks. Use a linked toolkit only where it fits; otherwise practise on paper or in your existing spreadsheet. This exercise does not require a download.

Example case / Risk & ControlsFrom a Control Description to a Defensible ConclusionOpen the case

Follow a fictional purchasing approval from its objective to operation, evidence and accountable action.

01 / Understand

Name the objective and the risk.

A fictional purchasing team wants commitments approved by the right authority before release. Write that objective before listing controls. Define the failure the control should prevent or detect.

What does an approval signature actually tell you?
Explore the reflection

Its meaning depends on the rule, authority, timing and evidence retained. The signature alone does not establish the entire process.

02 / Follow the Business

Locate the control in the process.

Follow the request, approval, release and subsequent changes. Identify the control owner, frequency, exception route and evidence expected at each handoff.

What should happen when the normal approver is unavailable?
Explore the reflection

Use an authorised exception or delegation route with clear boundaries and retained evidence. Do not assume informal substitution is permitted.

03 / Examine

Examine actual operation.

Compare the documented design with selected transactions and suitable event history. Understand timestamp meanings and the authority effective at the time. State the period and population the work covers.

Does a well-designed control prove it operated?
Explore the reflection

No. Design and operation require different evaluation. Gather evidence relevant to the conclusion you intend to make.

04 / Apply

Build an evidence-to-action record.

Create a working table with criterion, selected item, evidence source, result, limitation and follow-up owner. Keep exceptions separate from untested items and unresolved information gaps.

Can an unusual item be called fraud immediately?
Explore the reflection

A flag needs investigation. Establish facts, applicable criteria and explanation before characterising the issue.

05 / Connect

Connect accountability and improvement.

Discuss the evidence fairly, distinguish symptoms from causes and define a response that can be checked. Closure should reflect what has been verified and any remaining risk.

What should the follow-up show?
Explore the reflection

Identify the implemented change, the evidence reviewed and whether it addresses the issue within the scope tested.

A question to reflect on

How would an exception be recorded and escalated?

Use your own hypothetical example. Consider one assumption and one limitation.

Explore the reflection guide

A traceable working record keeps assumptions separate from facts and includes a control total or independently checked result.
